RESUMEN:

Un tópico clásico de la psicopatología alude a la dificultad de valorar el sufrimiento psicológico. El concepto “sentimientos negativos” (S.N.) originario de la psicopatología centroeuropea engloba componentes que los autores del presente artículo consideran útiles indicadores clínicos del sufrimiento psíquico. Dichos S.N. se refieren a afectos y emociones dolorosas que acompañan el día a día del sujeto que los padece. Así, los S.N. de soledad suelen asociarse a otros de desamparo y tristeza. Al decir de M.A. Mondelli es la “sensación de que algo falta y provoca ansiedad por una ausencia”. El S.N. denominado vacío emocional o sensación de no tener nada dentro es un trastorno de la vivencia del yo donde el propio cuerpo o también los propios pensamientos, sentimientos, etc. se sienten como extraños. Otro S.N. habitual es la culpa que puede ser intrapunitiva (culparse uno de lo ocurrido) o extrapunitiva (culpar a los demás de lo ocurrido, incluso de nuestros propios males). Los S.N. de utodestrucción aluden a una vivencia autolítica y tienen como consecuencia frecuente el aislamiento, incluso la conflictividad relacional. El S.N. de inescapabilidad emocional corresponde a sentirse atrapado/a en una situación generalmente relacional, sin visos de poder superar el problema. Finalmente la recreación de sentimientos negativos se refiere a pensamientos repetitivos de tinte afectivo negativo: tristeza, culpa, ansiedad, angustia y sentimientos análogos que generan insatisfacción. Paradójicamente el sujeto parece recrearse en ellos de forma masoquista, sufriendo y complaciéndose al mismo tiempo. Investigación: El dolor y el sufrimiento no se pueden medir pero si cualificar y valorar clínicamente. Se realizó un estudio que tenía un doble objetivo: observar la morbilidad de los sentimientos negativos dentro de la población general y cualificar la naturaleza de dichos sentimientos negativos dentro de una población diana de sujetos afectos de dicha nosología. Material y método: El instrumento de evaluación fue el Inventario IRIDS 100 de Sirvent y Moral, 2004 y –dentro del mismo- la escala de sentimientos negativos de dicho inventario para medir el sufrimiento mediante las subescalas de soledad, vacío emocional, culpa, autodestrucción, inescapabildad emocional y recreación de sentimientos negativos. A dicho propósito se estudiaron 78 sujetos diagnosticados de dependencia emocional comparándolos con 311 sujetos de grupo control (población general). Resultados y conclusiones. Los sentimientos negativos representan un excelente indicador de sufrimiento psicológico y el instrumento aplicado, el Inventario IRIDS 100 de Sirvent y Moral, 2004, se revela especialmente útil para clínicos, prácticos e investigadores. Epidemiología: La población general tenía una tasa de sufrimiento en torno al 10% y ciertos factores estaban próximos al 20%, cifra denotativa de un elevado nivel de afección general. La población diana elegida (dependientes emocionales que en su mayor parte no recibían tratamiento psiquiátrico ni psicoterapia alguna) tenía una tasa de 75-80% de sufrimiento psicológico. El porcentaje que lo sufre con máxima intensidad es superior al 30% entre los dependientes emocionales e inapreciable en el grupo control. Como elemento de discusión queda de manifiesto la enorme morbilidad tanto del sufrimiento psicológico como de los factores que lo componen.

ABSTRACT:

One of the classical topics of psycopathology alludes to the difficulty of measuring psychological suffering, full with subjective elements and – in consequence – complicated to target. The concept «negative feelings» (N.F.) coming from the Anglo-Saxon literature includes components that the authors of the present article consider to be useful clinical indicators of psychic suffering. Such N.F. refers to affections and painful emotions that go day after day with the subject who suffers them. From the nosologic perspective they can be located by property inside the depressive sphere by the dysthymic nature of the same ones, though also they appear in other processes (of nature mainly neurotic). This way, the N.F. of loneliness uses to be associated with others of abandonment and sadness. According to M.A. Mondelli is the «sensation that something is missing and provokes anxiety for an absence». The N.F. named emotional emptiness or sensation of not having anything inside is a disorder in the experience of oneself where the own body or also the proper thoughts, feelings, etc. feel as strangers (alienation – despersonalization with loss of motivation and the interest for what is happenning). Another habitual N.F. is the fault that can be intrapunitive (blame oneself for what happens) or extrapunitive (blame the others for what happens, even for our own wrongs). The N.F. of self-destruction alludes to an autholitic experience and takes the isolation as a frequent consequence, even the relational conflict. The S.N. of emotional unescapability is to feel trapped in a generally relational situation, without being able to overcome the problem. Finally the recreation of negative feelings refers to repetitive thoughts with affective negative characteristics: sadness, fault, anxiety, distress and similar feelings that generate dissatisfaction. Paradoxically the subject seems to enjoy them in a masochistic form, suffering and taking pleasure at the same time.
A study was realized which had a double objective: to observe the morbidity of negative feelings inside the general population and to qualify the nature of those negative feelings inside a population target of subjects affected by this nosology.
Material and method: The instrument of evaluation was the IRIDS 100 Inventory by Sirvent and Moral, 2004 and – inside it – the scale of negative feelings of the inventory to measure the suffering by means of the subscales of loneliness, emotional emptiness, fault, self-destruction, emotional unescapability and recreation of negative feelings. For that purpose 78 subjects diagnosed of emotional dependence were studied comparing them with 311 subjects of control group (general population). Results and conclusions. Negative feelings are an excellent indicator psychological distress and applied instrument, the Inventory IRIDS 100 of Sirvent and Moral, 2004, proves especially useful for clinicians, ractitioners and researchers. Epidemiology: The general population had a rate of suffering around 10% and certain factors were close to 20%, figures reflect a high level of general condition. The target population chosen (emotional dependent mostly received no psychiatric treatment or some psychotherapy) had a 75-80% rate of psychological distress. The percentage that suffers maximum intensity is above 30% for emotional and priceless dependent in the control group. As a discussion item is revealed both the enormous morbidity of psychological distress and the factors that compose it.
